National Healthcare Risk Management Week
June 19-23, 2006 Thursday, September 8th from 12:00 1:30 p.m. One of the six interventions of the IHI 100k Lives Campaign is to reduce adverse drug interactions through medication reconciliation. Ineffective medication reconciliation causes up to 50% of the medication errors which occur at admission and up to 20% of future adverse drug events. The Archives of Internal Medicine February 2005 issue reported that medication reconciliation will improve patient pharmaceutical care by reducing the number of omissions in medications and reduce the number of errors attributable to wrong dose, drug or frequency as patients move between locations. Medication reconciliation is one of the six interventions in the IHI 100k Lives Campaign. It is also a requirement for JCAHO 2006 as a patient safety goal. In this presentation you will hear from a JCAHO pharmacy surveyor about the requirements from JCAHO and how you will be surveyed based upon this patient safety goal. You will also have the opportunity to learn from two hospitals that have started medication reconciliation and what they are learning. From these hospitals you will hear about leading practices in Washington as well as on a national level. Focus will be given to strategies that can work in urban and rural hospitals.
